WAIVER AND RELEASE OF LIABILITY

1. Acknowledgment of Tutoring Environment

I understand that my child will be receiving tutoring services in a private home environment. I acknowledge that reasonable precautions will be taken to ensure the safety and well-being of my child during the scheduled tutoring sessions.

2. Assumption of Risk

I am aware that any time a minor is present in a private residence, there are potential risks. I voluntarily assume full responsibility for any risks, injuries, or damages, known or unknown, that might arise during or as a result of my child’s participation in tutoring sessions.

3. Health and Safety

I confirm that my child is physically and mentally fit to attend tutoring sessions. I agree to notify the tutor of any medical conditions, allergies, or special needs that could impact my child’s safety or participation.

4. Release of Liability

I hereby release, waive, and discharge Kristen Gasparino, the property owner, and any associated parties from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury that may occur while my child is attending tutoring sessions at the above-mentioned location.

5. Drop-Off and Pick-Up

I agree to ensure my child is dropped off and picked up promptly at the agreed-upon times. I understand that supervision is only provided during the scheduled tutoring session and not before or after.

6. Emergency Contact and Consent

In the event of an emergency, I authorize the tutor to seek medical care for my child if I cannot be reached.
